To get supplies from Access Point Mail Order, you must first complete our sign-up form in Microsoft forms.
Once you hit submit, a page saying "Your response was submitted" will load with a link to our Order Form, hosted in Google forms. Click that link to place your order with us and get supplies and educational info shipped right to you!

STEPS TO SIGN-UP & Get Mail Order
1. Select your region and choose to sign up no contact or with contact
2. Complete and submit your Sign-Up Form in Microsoft Forms and click the link to our order form
3. Complete our order form in Google Forms to select your supplies and educational materials
